Pulse STT keyword boosting and punctuation controls

Pulse STT now accepts punctuate and capitalize query parameters on the WebSocket handshake. Both default to true. Set to false for domains where consistent post-processing is preferred.

Keyword boosting documentation now describes the supported INTENSIFIER range (0–20, recommended 6). Values above 10 increase the risk of hallucinated keyword matches.
